THCS – Better care close to home: Enhancing primary and community care

The European Partnership on Transforming Health and Care Systems (THCS) brings the opportunity to coordinate and optimize research and innovation efforts in Europe and its partner countries supporting the much-needed transformation of health and care systems.

Under this umbrella, the third Joint Transnational Call for proposals “Better care closer to home: Enhancing Primary and Community Care” aims to improve primary and community health and care systems, providing policymakers with knowledge and tools to manage transitions within the primary and community care sector. Projects funded under this call will deliver promising financial, organizational, and practice-based service innovations that promote the transformation of health and care systems and contribute to faster exchange of best practices across different countries and regions.

Requirements

• The consortium must include at least three (3) eligible partners from three different countries
  whose funding organisations participate in the call. At least two (2) members of the consortium
  should be legal entities from two different EU Member States or Horizon Europe associated
  countries.
• Maximum number of partners eligible for funding is nine (9).
• Maximum two (2) eligible partners from the same country.
• The project coordinator must be eligible for funding by a regional/national funding
  organisation participating in the call.
• Maximum of two (2) collaborators per consortium are permitted. Collaborators do not
  count towards the maximum number of partners.

Application

Joint proposals consist of two parts:

• The pre-proposal form to present mainly the description of the planned work.
• The electronic submission tool to provide particularly individual partner information and financial plans.
